This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review carefully. The most common reason why we use or disclose your health information is for treatment, payment or health care operations. Examples of this may be requested directly through our office. Uses & Disclosures for other reasons without permissions. ^when a state & federal law mandates that certain health information be reported for specific purpose ^for public health purposes, such as contagious disease reporting, investigating or surveillance and notices to and from the federal Food & Drug Administration regarding drugs or medical devices ^uses & disclosures for health oversight activities ^disclosures for judicial & administrative proceedings ^disclosures for law enforcement purposes ^ disclosure to a medical examiner to identify a deceased person ^uses or disclosures for health related research ^uses or disclosures to prevent a serious threat to health or safety ^uses or disclosures for specialized governmental functions ^disclosures of de-identified information ^ disclosures relating to worker's compensation programs ^ disclosures of a "limited data set" for research, public health, or health care operations ^incidental disclosures that are an unavoidable by-product of permitted use & disclosure ^disclosures to business associates who perform health care operations for us & respect the privacy of your health information ^disclosures to other entities authorized by Eye Designs for such purposes as satisfying independent accounting firms, lenders, attorneys, auditors as required by Eye Designs in the ordinary course of business to firms engaged by Eye Designs to improve its business & collections and/or other such entities that Eye Designs authorizes. Unless you object, we will also share relevant information about your eye care with your family who are helping you with your eye care. Appointment Reminders: we may call, write or email you for the following: scheduled/routine appointments, notify you of treatments or services. Unless you tell us otherwise, we will mail you and appointment reminder, post card, or leave a message on your answering machine or with someone who answers your phone if you are not home. Your rights regarding your health information: The law gives you rights regarding your health information. You can: ^ask us to restrict uses/disclosures for purposes or treatment (except emergency treatment), payment or health care operations. We do not have to agree with this, but if we agree we must honor your restrictions. ^as us to communicate with our in a confidential way, we will accommodate these requests if they are reasonable, and if you pay us for any extra cost ^ask to see or get photocopies of your health information ^ask us to amend your health information if you think that it is incorrect or incomplete. ^get a list of disclosures that we have made of your health information within the last six years. By law, the list will not include: disclosures for purpose of treatments, payment or health care operations, disclosures with your authorizations, incidental disclosures, by law & other limited restrictions. You may receive one per year at no extra charge ^get additional paper copies of this Notice of Privacy Practices upon request. To request disclosures, confidential communications, photocopies, amendments if any, or any other disclosures, requests must be made in writing to the listed address, email, and fax to the contact person listed at the beginning of this notice. We have 30 days to comply, by law, we can have a 30 day extension of the time if we send you a written notice of this extension. Legal requirements: Eye Designs is required by law to maintain the privacy of your protected health information. We are required to abide by the terms of this notice and reserve the right to change this notice.
